摘要： Two incidents and a near miss, each one precipitated by an unplanned shutdown, are reviewed. In every case, a lack of understanding of the process control and / or chemistry led to an uncontrolled reaction that resulted in process equipment damage and, in one case, injured several people. The incidents are described in detail, and some general lessons are drawn from them.
